Semaglutide Injection (Ozempic/Wegovy)
Also called Ozempic · Wegovy · Sema
Semaglutide can reduce hunger and food intake, helping people lose weight. Wegovy injections are prescribed for weight management; Ozempic injections treat type 2 diabetes. Both are taken weekly, with gradual dose increases to help manage digestive side effects.

Product label- Dose
- Start: 0.25 mg for 4 weeks. After gradual titration: 1.7 or 2.4 mg maintenance.
- How often
- Once weekly, under the skin
- How long
- Long-term treatment; no fixed cycle
Maintenance follows gradual dose increases; 2.4 mg is label-recommended. Selected adults may use 7.2 mg only after at least 4 weeks tolerating 2.4 mg when additional weight reduction is clinically indicated.

Product label- Dose
- Start: 0.25 mg for 4 weeks, then 0.5 mg. Blood-sugar maintenance: 0.5, 1 or 2 mg.
- How often
- Once weekly, under the skin
- How long
- Ongoing treatment; no fixed cycle
Further increases depend on blood-sugar needs after at least 4 weeks at the current dose. For type 2 diabetes with chronic kidney disease, the label specifies 1 mg after at least 4 weeks at 0.5 mg.

One dose · modeled timing
Why weekly injections build on each other
100% = the model’s peak, not the dose left or a blood-test result.
About this curve
Some semaglutide remains when the next weekly injection is due. This one-dose example shows why later doses overlap.
- It rises gradually
- The injection is absorbed over time rather than entering the blood all at once.
- It fades gradually
- Drug from previous injections is still present as a regular weekly schedule continues.
Fixed study-model example in adults with type 2 diabetes. First-dose timing is extrapolated from an ongoing-treatment model. The curve explains overlap; it does not predict appetite or weight loss.
Model assumptions and sourcesOzempic and Wegovy: the difference
Both contain semaglutide, but their treatment goals and devices differ. Wegovy injections are used for weight management and certain other conditions; Ozempic injections are used for type 2 diabetes and specified heart and kidney risks. Their pens and dosing instructions are not interchangeable.
In STEP 1, adults with obesity or overweight and a weight-related condition, without diabetes, lost an average 14.9% of starting weight after 68 weeks with semaglutide 2.4 mg weekly plus lifestyle support, versus 2.4% with placebo. That result does not describe every dose or a lean bodybuilder’s cut.
For lifters: easier dieting versus eating enough
Bodybuilding discussions describe easier calorie control when cravings otherwise derail a cut. Other reports describe struggling to eat enough around heavy training. Less interest in food does not by itself show that a diet supports recovery or muscle retention.
In the STEP 1 body-composition substudy, adults with overweight or obesity lost both fat and lean mass, although body composition improved overall. The study did not establish what happens to lean, resistance-trained bodybuilders. Scale weight and appetite alone cannot show how much muscle was preserved.
Starting out and judging progress
The visible dose guide separates introductory and maintenance amounts. Later instructions depend on the product, treatment goal and tolerability. Early weight change cannot be compared fairly with a year-long trial result; the older 2.4 mg trial also does not describe the newer 7.2 mg option.
Hunger is not a blood-level test. In a 60-week study, people still ate less at laboratory meals even when later appetite-rating differences from placebo were unclear. Feeling hungry does not alone show treatment failure or that another injection is due.
Which side effects matter?
Digestive effects are common, especially during increases. Persistent vomiting, inability to keep fluids down or severe, persistent abdominal pain need medical attention. Relevant precautions include gallbladder disease, certain diabetes-drug combinations and diabetic eye disease.
The labels exclude people with a personal or family history of medullary thyroid carcinoma or MEN 2. Discuss pregnancy plans and planned anesthesia with the prescribing team.
How does it compare with tirzepatide?
A direct 72-week trial in adults without diabetes found greater average weight loss with tirzepatide at the tested doses. Both caused digestive effects. Treatment goals, tolerability, access and cost matter alongside that result; label milligrams are not a potency comparison.

How to read this chart
This is a study-based example of how semaglutide levels change over time. Try different doses or timing to compare the curves.
The calculation uses the same assumptions for everyone. It can't predict your own blood levels or tell you what dose to take.
Study setup and sources
Semaglutide: drug concentration in plasma (nmol/L).
Fixed reference profile: Published reference: 85 kg white, non-Hispanic female under 65 with type 2 diabetes and normal renal function. Subcutaneous semaglutide solution in the SUSTAIN phase III program; Subcutaneous. Fixed reference weight: 85 kg.
Changing dose or schedule explores a scenario; it does not personalize the reference profile or establish a studied regimen.
Input dose basis: Milligrams of semaglutide, not solution volume, syringe units or a salt/product conversion.
Source exposure scope: Published phase III maintenance groups were 0.5 and 1 mg weekly. Other amounts or timings are mathematical extrapolations of this fixed reference.

- Abdominal administration; all reference covariate factors and individual random effects fixed at their reference values.
- The source 1 mg maintenance-group clearance is held fixed when the entered amount changes; dose-group and other covariate effects are not implemented.
- Apparent clearance and volume already incorporate bioavailability; no additional availability factor is applied.
- Zero pre-existing semaglutide before the entered dose history; first-order absorption and linear elimination.

- This is a fixed reference calculation, not an individual blood-level prediction or the full published covariate model.
- The source used sparse steady-state samples and fixed absorption. Initial-dose peak timing is a model extrapolation, not a verified patient-specific endpoint.
- Oral semaglutide, compounded preparations and formulation changes are outside this model; approval of a product does not validate a different formulation.
- Concentration does not predict weight loss, glucose response, tolerability or a safe dose. A clearance threshold is not a safety or testing guarantee.

Common questions
Does feeling hungry before injection day mean semaglutide has worn off?
No. Semaglutide remains in the body between weekly doses, and the feeling of hunger does not directly measure its level or benefit. A recurring problem is something to review with the prescriber rather than using hunger alone to move the injection earlier.
Why might there be little change at 0.25 mg?
The injection labels use 0.25 mg as an introductory dose for four weeks. Large trial weight-loss results came after gradual escalation and many months of treatment, so they are not a fair expectation for the starting month.
Can I stop once I reach my goal weight?
Maintenance deserves a plan. In the STEP 1 extension, participants regained about two-thirds of their prior loss over the year after semaglutide and structured lifestyle support stopped. That group average does not predict one person’s course, but it makes follow-up after stopping important.

Notes on the quick facts
- What people seek
- Appetite and weight responses vary; the large weight-management trials studied months of treatment alongside lifestyle support.
- Brand matters
- Their approved uses, devices and maintenance doses differ. This guide covers the injections, not tablets.
- Common trade-offs
- Digestive effects are often more noticeable during dose increases. They are side effects, not a target for successful treatment.

- 2. Ozempic injection US prescribing information — May 2026
Type 2 diabetes and specified cardiovascular/kidney indications; weekly injection, introductory dose, approximately one-week half-life and 4–5-week steady-state exposure. Not a weight-management indication.
