Trulicity or Ozempic?

Ozempic led to greater average weight loss and blood-sugar improvement than Trulicity in a direct trial. Both are weekly diabetes injections; neither the dose number nor the trial average can decide which fits you.

Trulicity contains dulaglutide. Ozempic contains semaglutide, also used in the separate weight-management product Wegovy. Side effects, treatment goals and reliable access belong in the comparison.

Dosing and titration

0.75 mg weekly

Trulicity starting dose

May rise to 1.5 mg, then in 1.5 mg steps after at least four weeks at the current dose, up to 4.5 mg weekly.

0.25 mg weekly

Ozempic starting dose

First four weeks, then 0.5 mg. If needed, 1 mg and then 2 mg follow at least four weeks at each preceding dose.

These are product-label schedules for ongoing care, not short cycles. Ozempic’s 0.25 mg is an initiation dose. Wegovy has separate instructions. Sources: Trulicity label, Ozempic label.

Escalation is gradual and follows the prescribed product’s instructions. The reason to review a dose is the treatment goal and tolerability—not simply whether a simulated curve has reached a plateau. Switching requires a prescriber-defined starting dose and timing; there is no universal equal-milligram substitution or mandatory washout that fits every situation.

Weight loss results

SUSTAIN 7 was a 40-week, randomized, open-label study in adults with type 2 diabetes taking metformin. It compared two dose pairs, not the highest doses available today.

The graphic separates the actual trial pairs. The lower-dose pair lost 4.6 versus 2.3 kg on average; the higher-dose pair lost 6.5 versus 3.0 kg. The same study is more useful for this question than comparing results from unrelated trials. Primary report.

The trial also favored semaglutide for average HbA1c reduction. It cannot directly tell us how dulaglutide 4.5 mg compares with an obesity-dose semaglutide regimen, because those were not the arms tested.

Side effects compared

Both can cause nausea, vomiting, diarrhea and other digestive symptoms. In SUSTAIN 7, gastrointestinal disorders occurred in 43% versus 33% in the lower-dose semaglutide/dulaglutide pair, and 44% versus 48% in the higher-dose pair. That does not support a simple claim that one is always easier on the stomach. Trial results.

Both labels carry warnings about pancreatitis, dehydration-related kidney problems and other clinically important adverse effects. Severe or persistent abdominal pain, repeated vomiting or inability to keep fluids down needs medical advice. Low blood sugar is a particular concern when combined with insulin or a sulfonylurea.

Half-life and pharmacokinetics

The approximately five-day and one-week half-lives help explain why both injections can be given weekly: some of a previous dose remains when the next arrives. Exposure builds over several weeks.

A longer half-life does not automatically mean greater weight loss, fewer adverse effects or an equivalent dose. A comparison plot shows the selected models; it cannot rank clinical effectiveness from line height, especially when different molecules have different concentration scales.

Cardiovascular and kidney effects

The approved claims are product- and population-specific. Trulicity includes cardiovascular-risk reduction in adults with type 2 diabetes and established cardiovascular disease or multiple risk factors. Ozempic includes cardiovascular benefit in specified adults with diabetes and a kidney-outcome indication in adults with type 2 diabetes and chronic kidney disease. Do not transfer every claim to every GLP-1 medicine or every patient.

Frequently asked questions

Do both cause thyroid tumors in people? Their boxed warnings arise from rodent C-cell tumor findings; human relevance is not established. Both are contraindicated with a personal/family history of medullary thyroid carcinoma or MEN2.

Which fits weight management? Check a product with the appropriate indication, along with medical history, access and tolerability. Trulicity’s diabetes-associated weight change is not an obesity indication.

Can they be used together? They are overlapping GLP-1 therapies, not complementary ingredients to combine based on a plot. Discuss changes with the prescriber.
