Tirzepatide (Mounjaro)

Also called Mounjaro · Tirz · Zepbound

Tirzepatide helps reduce appetite and supports weight loss or diabetes treatment. In the US, Zepbound is the weight-management brand and Mounjaro treats type 2 diabetes. Both are weekly injections, with different dosing instructions.

Product label
Dose
Start: 2.5 mg for 4 weeks, then 5 mg. After gradual increases: 5, 10 or 15 mg maintenance; maximum 15 mg.
How often
Once weekly
How long
Long-term treatment; no fixed label course

Further increases use 2.5 mg steps after at least 4 weeks at the current dose, considering response and tolerability. The 2.5 mg dose is introductory, not maintenance.

Why weekly tirzepatide doses overlap

Tirzepatide (Mounjaro): a single-dose model illustration The line rises to its own model peak, then falls over 28 days. 100 percent means this model's peak, not the amount of a dose remaining. Fixed study-model example: 90 kg adult woman, BMI 32.5. 0% 50% 100% Model peak Day 0 Day 14 Day 28

100% = the model’s peak, not the dose left or a blood-test result.

About this curve

Some drug remains when the next injection is due. Repeated weekly doses gradually build toward a recurring pattern.

A dose fades over days
The drug is absorbed after injection and removed gradually.
Earlier doses still contribute
A consistent weekly schedule builds toward a repeating pattern over several weeks.

Fixed study-model example: 90 kg adult woman, BMI 32.5. The curve is normalized to this model’s peak. It explains timing, not the percentage of a dose remaining, appetite strength or expected weight loss.

What changes do people hope to notice?

People seek less hunger and weight loss, or better blood-sugar control when they have type 2 diabetes. The useful measure of progress depends on the treatment goal; feeling no hunger at all is not the goal.

In SURMOUNT-1, adults with obesity or overweight and a weight-related complication, without diabetes, received tirzepatide alongside lifestyle support. Average weight loss at 72 weeks was 15.0%, 19.5% and 20.9% in the 5, 10 and 15 mg groups, versus 3.1% with placebo. These results followed gradual increases and months of treatment, not a starting-dose test.

For lifters: losing weight while keeping training on track

Physique discussions focus on easier cutting, muscle retention and whether reduced appetite makes eating around training difficult. Some describe much less food preoccupation; others still report cravings despite weight loss. Hunger alone is a poor measure of body-composition progress.

In the SURMOUNT-1 body-composition substudy, adults with obesity or overweight lost both fat and lean mass. Lean mass includes more than skeletal muscle, so this is not a personal muscle-loss forecast. The study did not establish a muscle-preserving regimen for lean bodybuilders.

Mounjaro or Zepbound: what is different?

Both contain tirzepatide. In the US, Mounjaro treats type 2 diabetes; Zepbound is used for weight management and has a separate sleep-apnea indication in adults with obesity. Other countries may license them differently.

Their dose instructions differ: Zepbound has a specified introductory step, while Mounjaro’s current US label makes increases conditional on needing more blood-sugar control. The visible guide separates them. A shared ingredient does not make every device or instruction interchangeable.

Which side effects deserve attention?

Nausea, diarrhea, constipation and vomiting are common, especially during increases. Persistent vomiting, inability to keep fluids down or severe, persistent abdominal pain need medical attention. Insulin or sulfonylurea users also need a plan for low-blood-sugar risk.

The labels advise non-oral contraception or additional barrier protection for four weeks after starting and after each increase. They also cover gallbladder and pancreatic problems, severe stomach-emptying problems, and the medullary thyroid cancer/MEN 2 contraindication.

What happens after stopping?

In SURMOUNT-4, people switched to placebo after initial tirzepatide treatment regained weight over the following year, while those continuing lost more. Maintenance needs its own follow-up plan. A short cutting cycle or a blood-level chart cannot answer that question.

How to read this chart

This is a study-based example of how tirzepatide levels change over time. Try different doses or timing to compare the curves.

The calculation uses the same assumptions for everyone. It can't predict your own blood levels or tell you what dose to take.

Study setup and sources

Tirzepatide: drug concentration in plasma (nmol/L).

Fixed reference profile: Constructed adult female reference with type 2 diabetes: 90 kg and BMI 32.5 kg/m²; not a published average participant. Subcutaneous tirzepatide in the published pooled-study model, using its estimated relative-bioavailability scenario; Subcutaneous. Fixed reference weight: 90 kg.

Changing dose or schedule explores a scenario; it does not personalize the reference profile or establish a studied regimen.

Input dose basis: Milligrams of tirzepatide, not solution volume, syringe units or a product conversion.

Source exposure scope: The source pooled single and repeated subcutaneous dosing across phase I–III studies. Entered schedules are simulations, not treatment instructions.

Published two-compartment model: constructed 90 kg female reference

  • Body weight, sex and BMI stay fixed. Fat-free mass 49.9282 kg and fat mass 40.0718 kg are calculated from the published female equation.
  • Clearance and intercompartmental clearance scale with (90/70)^0.8; both volumes scale with (fat-free mass + 0.482 × fat mass)/70.
  • Effective availability is 0.8 × (1 − 0.181) = 0.6552, the supplementary model setting for pooled studies other than the phase-I absolute-bioavailability reference study (STUDYN=4). It is applied once.
  • Individual random effects are zero. No weight-loss trajectory or pre-existing tirzepatide is assumed before the entered dose history.

  • This is a fixed scenario from the published population model, not an individual prediction or a published mean concentration profile.
  • The source allowed body weight to change over time; this implementation freezes body size and does not simulate parameter variability or uncertainty bands.
  • The between-study availability term is model context, not evidence that every marketed, compounded or investigational formulation shares this availability.
  • Concentration does not predict treatment response, tolerability or a safe dose. A clearance threshold is not a safety or testing guarantee.

Common questions

Is 2.5 mg always just a starter dose?

For Zepbound, yes: the US label identifies 2.5 mg as initiation only and specifies 5 mg after four weeks. Mounjaro’s current US label differs: it starts at 2.5 mg and makes further increases conditional on additional blood-sugar control after at least four weeks.

Does being hungry before the next injection mean it has worn off?

Not necessarily. Tirzepatide remains between weekly injections, and hunger is not a direct blood-level measurement. Review a recurring problem with the prescriber instead of using one hungry day to select an earlier injection or larger dose.

Does tirzepatide produce more weight loss than semaglutide?

In SURMOUNT-5, adults without diabetes lost more weight on average with tirzepatide 10/15 mg than with semaglutide 1.7/2.4 mg over 72 weeks. That supports the comparison at those trial doses; it does not predict everyone’s result or compare newer 7.2 mg semaglutide.

Do Mounjaro and Zepbound need reconstitution?

The approved US products are supplied as liquid solutions with product-specific pen or vial instructions. A generic powder-mixing recipe does not apply, and the total amount in a multidose device is not the amount of one injection.

Notes on the quick facts

What people seek
Tirzepatide acts on the GIP and GLP-1 hormone pathways. The reason for treatment determines which outcomes and product instructions matter.
US brand distinction
The same ingredient does not make every indication, dose instruction or device interchangeable.
Common trade-offs
Digestive effects are often more noticeable during dose escalation. A larger dose is not automatically a better fit.
