Pramlintide (Symlin)

Also called Symlin

Symlin helps people using mealtime insulin control the rise in blood sugar after food. It may also help with fullness and weight, but its meal-by-meal use and insulin adjustments make it very different from a weekly weight-loss injection.

Dosage & duration

US label context for adults already using mealtime insulin.

Type 1 diabetes

Product label
Dose
15 mcg starting dose; titrated toward 30–60 mcg
How often
Immediately before major meals, under the skin
How long
Ongoing treatment

Gradual increases require tolerability assessment and a coordinated insulin plan.

Type 2 diabetes

Product label
Dose
60 mcg starting dose; may increase to 120 mcg
How often
Immediately before major meals, under the skin
How long
Ongoing treatment

Label defines major meals as at least 250 calories or 30 g carbohydrate.

These are micrograms, not milligrams. The label’s insulin changes and monitoring are integral to treatment.

Symlin and mealtimes

An after-meal glucose tool with extra coordination

  • After-meal blood sugar

Symlin helps limit the rise after food in people using mealtime insulin.

  • Meals shape the routine

Treatment is linked to qualifying meals rather than a weekly appetite schedule.

  • Insulin needs a plan

The combination can cause severe low blood sugar and requires clinician-led insulin adjustment.

What people use it for

Pramlintide replaces some actions of amylin, a hormone normally released with insulin. It slows stomach emptying and helps limit the after-meal glucose rise. This is add-on diabetes care, not a substitute for insulin.

Why the insulin plan matters

Symlin and mealtime insulin can cause severe low blood sugar together. The label calls for an initial 50% mealtime-insulin reduction, frequent glucose checks and subsequent clinician-led adjustment. Do not add it to an unchanged insulin routine based on a plot.

Meals, not a weekly cycle

The injection is linked to major meals rather than taken as a weekly appetite shot. Nausea and changes in food intake affect how well it fits daily life. Confirmed gastroparesis and hypoglycemia unawareness are contraindications.

How to read this estimate

Estimated from study endpoints; the curve between measurements is reconstructed. Compare amounts and timing in mg.

  • Observed extravascular concentrations already contain bioavailability; do not multiply by legacy F or add an independently assumed F.
  • Dose scaling and repeated-dose superposition use fixed parameters; the example dose does not calibrate the curve.
  • No added30–40%bioavailability multiplier; observed SC endpoint already contains it.
  • One-compartment reconstruction overestimates AUC by15.17%, so it is an endpoint approximation.
  • Fast peak needs sub-hour sampling to avoid an apparently tiny/missing peak on coarse weekly grids.

Common questions

Does Symlin replace insulin?

No. It is an add-on for eligible people using mealtime insulin.

Is this the same as cagrilintide?

No. Both relate to amylin, but their products, timing and evidence differ.
