What to expect from semaglutide

Semaglutide can reduce hunger and help with weight or blood sugar, depending on the product. The first month is a starting phase: longer-term results develop during continuing treatment, with gradual dose increases.

Wegovy, Ozempic and semaglutide tablets contain the same active ingredient but have different instructions. Start by identifying the exact product and form.

Wegovy injection · US adult label

A gradual start

  1. Weeks 1–4 0.25 mg
  2. Weeks 5–8 0.5 mg
  3. Weeks 9–12 1 mg
  4. Weeks 13–16 1.7 mg
Weight-management maintenance 1.7 or 2.4 mg weekly

2.4 mg is recommended. Tolerability can change the timing of an increase.

Every amount above is per weekly injection, not per day. Tablets, Ozempic and the selected-adult 7.2 mg option have separate details below. Prescribing information

Dose, frequency and how long treatment lasts

The opening timeline shows the usual Wegovy injection start. Each stage covers four weekly doses before the next amount, unless the prescriber adjusts the plan for tolerability.

The amount rises in stages; it is not a prescription to rush upward despite side effects. Treatment for weight management is ongoing when appropriate, rather than a fixed short cycle. These are injection instructions, not tablet or Ozempic instructions. Current label.

The current US label includes several presentations and indication-specific choices, including a 7.2 mg option when additional weight reduction is clinically indicated in adults who have tolerated 2.4 mg for at least four weeks. Older charts that describe 2.4 mg as the only possible adult maximum are incomplete. Use the prescribed product’s current instructions; never calculate a switch or missed-dose plan from half-life alone.

What the curve shows during titration

At each increase, new input overlaps with residual drug from earlier weeks. The chart can make that continuity visible. It does not show that a starting dose has “no effect,” that nausea is inevitable at a particular concentration or that a person must increase after exactly four weeks.

How much weight loss is realistic?

Semaglutide acts on appetite and glucose-regulation pathways. Product identity matters: injected and oral formulations have different absorption and administration instructions. A milligram of a tablet is not interchangeable with a milligram injected.

Why weekly doses build up gradually

TermWhat it means for the weekly injection
Half-lifeAbout one week: concentrations decline gradually rather than disappearing between doses
Time to peakUsually around 1–3 days after injection, not immediately at the needle event
Repeated dosingPrevious doses still contribute when the next dose is given
Output on the chartModeled drug concentration, not a measured blood level or a weight-loss forecast

Product PK is described in current Wegovy prescribing information. The compound page identifies the model used on this site.

Tablets or injections: what changes?

The form changes the routine

Compare Weekly injectionDaily tablet
Frequency One weekly input One daily input for labeled oral products
Administration Follow the specific injection-device instructions Follow tablet-specific fasting and water instructions
Absorption From tissue under the skin Strongly affected by formulation and how it is taken
The same milligrams do not mean the same exposure. Use the prescribed formulation’s instructions.

As of the current US label, Wegovy includes both injection and tablets. Product names, strengths and indications should be checked directly; old “oral semaglutide is only for diabetes” summaries may be outdated. Do not use a simple bioavailability percentage to convert one formulation into the other.

Nausea and other side effects to watch

Nausea, vomiting, diarrhea and constipation are relevant GI effects. Severe persistent abdominal pain or inability to keep fluids down needs assessment. Discuss insulin or other glucose-lowering medicines, pregnancy plans, retinopathy history and upcoming anesthesia with the treating team. The label’s contraindications include personal/family medullary thyroid carcinoma or MEN2; this is not a blanket statement that every thyroid condition is the same.

For compounded vials, concentration and measurement need particular attention. FDA has documented errors involving mg, mL and syringe “units.” FDA dosing-error alert

Reading your curve

Enter the correct formulation and the actual schedule, then check units and model notes. Compare patterns—overlap, delayed peaks and gradual decline—without interpreting a lower trough as a diagnosis or a reason to add an injection. See reading a PK curve.

Frequently asked questions

How long does semaglutide stay in the body?

Its half-life is about one week. Under a simple elimination calculation, five half-lives leaves about 3.1% of a starting amount, not zero. Follow product-specific clinical advice for stopping, switching or pregnancy planning rather than a calculated clearance date.

Why can side effects change even when the weekly dose stays the same?

Earlier doses can still contribute as exposure builds, but concentration is only one possible contributor. A curve cannot identify the cause of a symptom. Persistent vomiting, severe abdominal pain or difficulty keeping fluids down needs medical assessment.

Can I skip a titration step?

Dose changes belong with the prescriber. Escalation is intended to manage tolerability, and the actual product instructions allow treatment response and tolerability to affect the plan.

Does a weight-loss plateau mean the drug reached steady state?

No. Steady state describes a repeating concentration pattern; a weight plateau describes an outcome over time. The curve cannot diagnose a plateau or show that another dose increase is needed.
