Retatrutide: Weight Loss, Trial Doses and Side Effects
Why retatrutide is getting attention
Retatrutide produced large weight losses in trials: an average 25.0% in the 12 mg group over 80 weeks in the published TRIUMPH-1 analysis. It remains investigational, with no approved prescribing schedule.
Its three hormone targets are GLP-1, GIP and glucagon. The promising trial results do not establish that a research-market vial has the same identity, quality or effects.

Main weight assessment
Continued treatment was studied, not a short bodybuilding cycle.
Trial supervision included follow-up for benefit and adverse effects. These research amounts do not supply an approved starting or switching dose.
How much weight did people lose?
The opening graphic uses one consistent analysis across all three dose groups and placebo. Digestive events were the most common adverse events. The paper also examined knee-osteoarthritis and sleep-apnea outcomes in relevant subgroups. Published phase 3 trial
Why do other reports say 28.3% or 30.3%? Lilly’s earlier release reported 28.3% at 80 weeks under an efficacy analysis, which asks a different question about remaining on treatment. The 30.3% figure came from a selected 104-week extension. Neither should replace the 80-week intention-to-treat figure without its context. Lilly’s May 2026 release
What does the third receptor add?
GLP-1 and GIP are involved in glucose regulation and appetite-related pathways. Retatrutide adds glucagon-receptor activity, a pathway involved in liver metabolism and energy regulation. The combined molecule is studied as one treatment: a trial cannot attribute a fixed share of weight loss to each receptor, or prove that adding a separate drug would reproduce the effect.
The original discovery and early clinical paper describes the molecule and its receptor profile. “Three receptors” is a description, not a guarantee that it is the best treatment for every person.
What about nausea and heart rate?
The earlier 338-person trial reported average weight reductions of 24.2% with 12 mg and 2.1% with placebo at 48 weeks. GI effects were dose-related; heart rate rose in a dose-dependent pattern, peaked around week 24 and then declined. These are phase 2 observations, not proof of long-term cardiovascular safety. 2023 trial
Why is it studied once a week?
The phase 1b study in people with type 2 diabetes found an approximately six-day half-life, supporting weekly investigation. That does not establish a universal 80% bioavailability figure or make the plotter a dosing guide. A curve describes modeled exposure, not weight loss, appetite suppression or the timing of adverse effects. Urva et al.
Retatrutide vs tirzepatide (and semaglutide)
Semaglutide activates GLP-1 receptors; tirzepatide activates GIP and GLP-1; retatrutide adds glucagon activity. Their trial percentages cannot be ranked simply by selecting the largest headline: populations, duration, dose escalation and analysis differ.
There is direct obesity-trial evidence comparing tirzepatide with semaglutide, discussed in the tirzepatide guide. The studies summarized here do not provide a completed direct retatrutide comparison with either drug.
What remains uncertain before routine treatment?
Questions still include uncommon harms, longer-term outcomes and how benefits persist with continued or discontinued treatment. Product identity also remains a separate issue: trial results do not validate unapproved retail preparations.
Frequently asked questions
Is retatrutide approved?
The developer still describes retatrutide as investigational as checked in October 2026. Clinical trial publication does not itself constitute regulatory approval.
Which weight-loss number should I use?
For the published TRIUMPH-1 treatment-regimen analysis, the 12 mg group averaged 25.0% reduction at 80 weeks versus 3.9% with placebo. Other analyses and the longer selected extension answer different questions.
Does the six-day half-life tell me when it stops working?
No. Elimination, appetite effects, weight change and safety are different outcomes; a PK curve cannot determine a personal redosing or switching schedule.
